The review screened 528 records and included 45 studies: 19 randomized trials, 9 comparative studies, 9 nutraceutical studies, while 8 reported no significant effect. Importantly, its search ended in April 2024, so newer 2025 hypertension data were not included.
In a Cuban multicenter trial, 200 prehypertensive participants received placebo or 20 mg/day sugarcane-wax alcohols for 12 weeks. 44% reached an SBP reduction ≥10 mmHg versus 7% with placebo. The Grade I hypertension stratum later reported 74% versus 12%.

A German randomized trial found no significant LDL-C benefit at 10–80 mg/day, and other independent studies also reported null lipid results. This challenges the idea that every “Policosanol” product is clinically interchangeable.
Composition may contribute, but cannot explain everything. Policosanol is a mixture of long-chain primary alcohols, commonly including C24, C26, C28 and C30. Food Chemistry research showed source-dependent profiles and found some supplements containing less total Policosanol than claimed.

This is why Octacosanol is not synonymous with Policosanol: C28 may be a major constituent, but Policosanol is a mixture. Compositional consistency therefore matters.
